Intake Form Completed

This email is sent for more than one kind of form. Check which form the client completed, then follow the matching part below.
Part 1 — Patient Intake Form

Open the email with subject "Client intake form completed".

Click the client's name to open their Halaxy profile.

Check for any tasks or alerts to clear.

If there is a Halaxy task labelled iffu, mark it complete.

If there is a 'No intake form' alert, delete it.

Click Clinical Notes then open "Patient Intake Form".

Check if the client has used any Medicare rebates for the year. Go to question 3 and check the answer.

Send email template '10 Prior Appointments' + SMS template 'Email sent'. Stop here.

Go to the Funding tab → click ADD REFERRAL → set Limit to the number of rebates used → click Add prior appointments and set it to the same number → set Date to 01-Jan of the current year → click SAVE.

Go to the General tab → click Add Appointment in the top right. Enter the appointment details and press Save. Repeat this once for each prior appointment (e.g. 4 prior → do this 4 times).

Click General → click Edit next to the client's name and update the following:

Update the mobile number to the format: +61417746631.

Part 2 — Consent to Exchange Information Form - Clinical Notes

Let the Australian admin know via Slack.

In the client's Clinical Notes, find the form and read its Practitioner: line.

If it shows a therapist, leave it with them. If it shows Remote Therapy Admin or Automation Bot, click the pencil next to the form's title, choose the client's old therapist (from their last session, or their next booking if they have never had a session) and save.

Part 3 — Other - DASS, PCL, etc

Forward the email to the client's therapist.

In the client's Clinical Notes, find the form and read its Practitioner: line.

If it shows a therapist, leave it with them. If it shows Remote Therapy Admin or Automation Bot, click the pencil next to the form's title, choose the therapist on their next booking (or their last one if nothing is booked) and save.

Part 4 — Card Details

Let the Australian admin know via Slack.

Do not forward this to the therapist, and do not share it — it holds the client's payment details and nobody else needs them.

Part 5 — Medicare details

Open the client's Funding page.

Check the client's Medicare number is correct by clicking the circle button to verify it.

If applicable, claim any unclaimed Medicare invoices (open invoices → click 'Process Bulk Bill') and set a cc task for tomorrow.

Check for a "Check medicare details form completed + claim" or cmd task — if one exists, set it to today. If not, check for an upcoming booking. If there is one, create a cmd task and set to today.

Chase it up with the client — ask them to check their Medicare card and send the correct details, exactly as for an incorrect Medicare card on the intake form.

If the profile appears blank (no timezone, no intake form, no referral), search for the client's name in Halaxy.

Merge the two profiles. On the profile with the new booking, click 'Edit' in the General tab → click 'Merge' → search for the existing profile, making sure the date of birth matches → click 'Next' → click 'Merge'.

Once merged, edit the profile, delete any extra mobile numbers, and update the remaining number to the format +61417746631.

On the Funding tab, merge the claims.

Void the fee + add comment: DD/MM - 6+ month old invoice - voided. Continue to Step 10.

If the client has changed therapist:

Send 'Client Changing Therapist' WunderText template to the old psych. Do not do this if the last appointment was more than 1 year ago.

Set old psych Relationship to 'Discharged' on the client profile. Keep Access Type as 'Own Access Level'.

Delete the appointment that is furthest away.

Send the '1 Therapist Only' Halaxy template — fill in therapist name, appointment date, and appointment time (in client's timezone) where it says XXXX.

Go to Contacts → Fax Messages.

Click on a fax that is not attached to patient 'Joe Schmierer'.

Click 'LINK CLIENT, ORGANISATION, OR PROFESSIONAL CONTACT'.

Add 'Joe Schmierer' as the client and click SAVE.

Click the fax_document.pdf link to download it.

Email the file to admin@remotetherapy.com.au.

Move the email into the 'Unactioned referrals' folder.

Check the client's mobile number is correct by comparing it to the referral. If different, update it and resend the failed SMS.

If the above doesn't work, or the number is correct, email the GP clinic using the template 'GP Email — Incorrect Phone Number'.

Check for obvious errors in the email address (e.g. typos, incorrect .au suffix). If found, fix and resend the failed email.

If there are no errors, send the text template 'Email failed'.

Make a note of the date and sender name including the alphanumeric code.

Click 'View Message'.

Click the secure message(s) that match the correct date and sender name. There can be multiple.

Use this list when determining whether to charge a late cancellation fee.

Part 1 — Check Attendance Record

Only count cancellations that are the client's fault. The following do not count:
